This can happen when IIS server is installed before Visual Studio. My guess is you installed the 32bit package on a 64bit machine, or a 64bit package on the 32bit machine, and the DLL is failing to load. After starting the App Pool, the issue was resolved. I do have "Anonymous Authentication", "ASP.NET Impersonation", "Basic Authentication", "Forms Authentication", but no "Windows Authentication".
